Step 1: Your Date of Birth:
Enter your date of birth. Click on the box “I am at least 18 years old”. Click the “continue” button.
Please note that the person completing the registration must be 18 years of age or older to comply with the Child Online Protection Privacy Act. Parent’s registering their children should enter their date of birth in this section and the child’s date of birth throughout the rest of the registration process.
Select a Category:
Select “Ice Player”.
Select a State:
Select AZ from the drop down menu. Click the “continue” button.
Step 2: Who are you registering?
Select “Ice Player”. Click the “continue” button.
Step 3:
Individual Membership Registration Form:
Complete all required fields (required fields are flagged with a red *).
Read the Waiver Statement, then click the box next to “I agree to the above waiver”.
If you are registering more than one family member click on the “Register Someone Else” button. Once complete click on “Continue to Checkout”.
Cart Summary:
The cart summary will include a list of your completed registrations and the total amount that will be charged to your credit card. At this time you can edit an existing registration, delete a registration, register another player in your family or continue to checkout.
Step 4: Billing information:
Enter your credit card information.
Step 5: Registration Confirmation:
Please print and save your confirmation page. This information will also be e-mailed to the address provided during registration. Attached to that e-mail will be a copy of your online registration confirmation.
Please note that once you complete your online registration process, you are registered with USA Hockey, BUT YOU ARE NOT YET REGISTERED WITH THE PHOENIX POLAR BEARS. You will need to email your confirmation page to Mary Pesce, Registrar, mpesce@cox.net . We need this information to complete our team registrations
